The Swans came from a goal down to earn their second away win of the season in snowy conditions at the Hawthorns.
West Brom went ahead shorty after half-time when Marc-Antoine Fortune fired home after a corner wasn't cleared by the Swansea defence.
But on-loan Gylfi Sigurdsson inspired an immediate comeback as he slotted home Neil Taylor's cross.
And the turnaround was complete shortly after as Danny Graham slid in to finish Sigurdsson's cross from the right after a wonderful passing move.
The result moved Swansea City on to 30 points in their maiden season in the Premier League, with Brendan Rodgers' side eventually going on to finish the campaign in 11th place.
