An injury-time strike from Craig Beattie secured the Swans all three points in a thrilling contest at the Riverside.
Middlesbrough's Marvin Emnes scored the opener early on thanks to a Leroy Lita assist, before the visitors equalised through Nathan Dyer's well-taken goal.
Jonathan Grounds put Boro back in front before the interval as he got the final touch to Barry Robson's free-kick, while the Scot's corner fell kindly for Leroy Lita in the second half as the striker headed home to put the hosts 3-1 up.
However, the Swans fought back and when Julio Arca fouled Joe Allen inside the box, Scott Sinclair stepped up to narrow the deficit to just one goal.
Brendan Rodgers' side then hauled themselves level shortly after as Angel Rangel's shot rebounded to Ashley Williams who fired home from six yards out.
And with the clock ticking, Swansea grabbed victory in stoppage time as Craig Beattie hit a superb left-foot shot from 20 yards that flew into the bottom corner.
