The Swans were cruelly denied victory as former striker Steve Torpey struck in the last minute at Glanford Park.
The hosts took the lead after just six minutes when Billy Sharp headed home Matt Sparrow's corner, before the striker was replaced by Torpey soon after due to injury.
Andy Robinson got the Swans on level terms before the half-hour mark as he fired home a penalty after he had been brought down by Cleveland Taylor.
It got better for the visitors as striker Leon Knight took full advantage of a defensive mix up before slotting home.
But Torpey came back to haunt his former club as he rose to head a corner past Willy Gueret via the far post.
The Swans would end the season in the final play-off spot - beating Brentford in the semi-finals before penalty shootout heartbreak against Barnsley at the Millennium Stadium.
