Adrian Forbes' 89th-minute winner in front of almost 9,000 fans at the Vetch sent the Swans to the top of League Two.
Forbes, who had come off the bench 15 minutes from time, was left unmarked to head home a Leon Britton cross.
The home side enjoyed near constant pressure but Lee Trundle and Paul Connor missed decent opportunities to open the scoring.
Colin Larkin had Mansfield's best chance when he beat the offside trap, but his effort was well saved by home goalkeeper Willy Gueret.
